Please review the chart below to submit your 2024 - 2025 school year request. This form can be used for new hires, rehires, and transfers.

Please use your LAUSD email account to log in to access the form. 

All appointments for processing will be in a remote fashion; third party fingerprint will be required at the candidate's expense only when notified to complete.

Completion of the request will require attachment of the approved

• Request for Personnel Action (RPA)
• Nepotism Form
• Application for Issuance of Teacher Assistant Certificate.
• If this is for a Transfer, please upload the TA Transfer Form in lieu of the Application for Issuance Form.

Please have all forms ready as a PDF or electronic file to upload. Teacher Assistant onboarding requests must be entered by the Principal, SAA, or coordinator using their LAUSD Single Sign-on (full email with @lausd.net) and password to access the request form. If you are experiencing difficulty accessing the form, be sure to log out of all personal Google accounts and log in using your LAUSD Single Sign-on.  

Candidates must meet the following requirements and be enrolled in school to onboard as a Teacher's Assistant: Teacher Assistant Qualifications

In addition to the minimum requirements, per California Education Codes 10020, 44323, & 44926, Teacher Assistants must present a completed Application for Issuance of Teacher Assistant Certificate. This document verifies current enrollment in college*. In addition, TAs must maintain continuous college enrollment and meet all unit completion requirements to remain employed as a TA.
