Teacher Assistant HR Fomrs - New TA Processing Forms

To request to hire a qualified Teacher Assistant, the school site should request to hire the candidate by filling out the form at http://teachinla.co/TARequestToHire. The following forms will need to be approved and uploaded with the request to hire:
- Request for Personnel Action
- Approved Nepotism Form
Note that the following forms require candidate completion by other agencies prior to a virtual onboarding appointment:- Applications for Issuance of Teacher Assistant Certificate
- Tuberculosis Compliance Forms (Additional medical clearance is necessary for e/TK and Early Education positions.)
- Fingerprinting (at third-party agency; instructions will be provided)
- TA Assignments Packet (Virtual Onboarding)
- TA Payroll Packet (Virtual Onboarding)
All applicants will need to provide acceptable I-9 documents for employment authorization and identification; a list of acceptable documents for I-9 can be found here: I-9 Documents. Names on all documents must match exactly including any and all middle names/initials, additional surnames, or suffixes; original documentation showing proof of name change is required.Former US military service members will need to present DD214.Official transcripts are preferred documents for verification of minimum qualifications. Names on all documents must match exactly including any and all middle names/initials, additional surnames, or suffixes, or original documentation showing proof of name change is required.Due to the COVID-19 situation, electronic transcripts and verification of enrollment maybe accepted only if sent directly to Human Resources. Please communicate with HR staff prior to ordering any electronic documents.

Tuberculosis Compliance Forms
