Provisional Waiver Contract

A contract of an employee serving under a Special Education Waiver. This employee does not meet the requirements for an Emergency Permit, but is otherwise qualified to serve in a Special Education position. This contract must be renewed each year after a satisfactory evaluation (Form 1022) from the school administrator.
