Please note the following changes:  

1. Effective January 1, 2016, the Medical Provider Network (MPN) notice that was distributed with the District three-part mandatory employment notice poster is eliminated and must be taken down.
2. Also effective January 1, 2016, the state has revised the mandatory "Notice to Employees - Injuries Caused at Work" (poster DWC 7). This revised version now includes pertinent MPN information, and will replace the MPN notice.
3. Effective February 1, 2016, the District will implement a new Medical Provider Network for use by employees who are injured on the job. The new Network information must be reflected on the "Notice to Employees - Injuries Caused at Work" (poster DWC 7); therefore, the "Notice to Employees" must also be revised, resulting in two revised versions of this poster, one effective January 1, 2016, and the other effective February 1, 2016.
