    MetLife Plan Guide

MetLife is the plan administrator for basic and supplemental life insurance.
All eligible active employees automatically receive $20,000 basic life insurance coverage, underwritten by Metropolitan Life Insurance Company.
Optional life insurance with higher limits and additional benefits, including coverage for eligible dependents, is available and is paid for through monthly automatic payroll deductions.

In general, employees who work at least half of a full-time regular assignment in one classification are eligible.  However, other employees may qualify under certain circumstances.  View MetLife's Insurance Plan Guide for further information.

Eligible employees are automatically enrolled.  To designate beneficiaries, fill out the Beneficiary Designation Form or contact MetLife directly.

Contact information is provided at the top of this page.  Forms are found under the "Life Insuarance" section of the Forms and Publications page.

Optional life insurance is a voluntary benefit that allows the purchase a greater level of life insurance than the $20,000 basic life coverage provided by the District.  It also allows insurance coverage for eligible dependents including children and spouses/domestic partners.

Optional life insurance also features an accidental death and dismemberment (AD&D) benefit for eligible employees and their spouse/domestic partner.  There are no AD&D benefits for dependent children.

The AD&D benefit doubles the death benefit payable if the covered individual dies as a result of a covered accident.  It also pays benefits for serious injuries as a result of a covered accident.

Optional life insurance also provides accelerated life benefits if covered individuals become terminally ill (a life expectancy of less than 12 months).  This benefit may help defray the expenses associated with the condition and may protect other assets.  It also ensures the availability of funds for the beneficiary upon the death of the covered individual.

Costs depend on the coverage and payouts selected by the employee.  Contact MetLife for details (contact information provided at the top of this page).

Should you leave LAUSD for any reason, and your Supplemental and Dependent Term Life insurance under this plan terminates, you will have an opportunity to continue group term coverage (“portability”) under a different policy, subject to plan design and state availability.  Rates will be based on the experience of the ported group and MetLife will bill you directly.  Rates may be higher than your current rates.

Generally, there is no minimum time for you to be covered by the plan before you can take advantage of the portability feature.  Please see your certificate for specific details.  Please note that if you experience an event that makes you eligible for portable coverage, please call a MetLife representative at 1-888-252-3607 or contact your employer for more information.

To view related documents, please visit the "Life Insurance" section of the Forms and Publications page.

If enrolled in optional life insurance, plan will be automatically switched to a special "decreasing term" policy.  Coverage will equal 50% of active employee coverage and will decrease by 10% (of initial coverage) every year until minimum coverage amount is no less than $5,000.

Premium payments must be remitted directly to MetLife.
